Who Can Apply

Seniors

Individuals must be 60 years old or older to qualify. Seniors should complete an application and are required to present a current photo ID that includes a date of birth. There are several ways to show eligibility:

1. Medicare card

2. A state-issued picture identification card that verifies your age such as…

  • Drivers License
  • State of Michigan ID card
  • Passport

Persons with Disabilities

All persons with disabilities should complete an application and submit proof of disability. There are two ways to show eligibility:

1. Proof of Social Security disability (SSD) award letter from the federal government

2. Part C: Disability Certification must be completed and signed by a physician or certified human service professional such as a social worker.

All Applicants:

Must show a state-issued picture identification card that verifies your age (photo ids such as a drivers license, state of Michigan ID card, or passport) is required to process all applications.
